Results

This project is based on a public / private collaboration between the Louis-Constant Fleming Hospital of Saint Martin and the LEPERS laboratory. It associates 4 smaller countries in the region of Saint Martin: Sint Maarten, Anguilla, Saba and Montserrat. It aims at organizing the networking of antiretroviral prescribers and monitoring HIV resistances to treatments in the sub-region. It should eventually allow the monitoring of other viral diseases.

160 genotypes of HIV patients from St Martin, St Maarten and Anguilla were analyzed. They have demonstrated a high rate of resistance to NNRTIs in naive and pre-treated PLHIV.

The preferential use of NNRTIs first line, precarity and stigma, limited recourse to patient's therapeutic education (PTEs) could explain these results.

Recommandations

A generalized PTE program, early genotyping of virological failure as well as naive patients and increased social assistance could help contain the emergence and transmission of resistance.

This project has brought to light a resistance to HIV non-nucleoside in the sub region that should allow to shift the pattern of first prescriptions and avoid taking combinations of drugs which are insufficiently efficient, costly and generators of other resistances, for the individual and collective benefit...
